Parent Support Grant Program Terms
These terms apply to organizations and professionals ("Applicants") that apply for or receive a ThriveGuide Parent Support Grant (the "Grant"). They supplement our Terms & Conditions and Privacy Policy. If there is a conflict, these Grant terms apply to the Grant.
1. Program administration. The Grant is a private program funded and administered by Health Innovation Platform, Inc. ("ThriveGuide"). It is not a government program, and ThriveGuide is not affiliated with any government agency.
2. Eligibility. Applicants must be organizations or professionals serving parents and caregivers of young children who have recently been identified as needing early intervention or are awaiting services. ThriveGuide may verify any information provided and may decline or withdraw an award where eligibility cannot be confirmed or information is inaccurate.
3. The Grant. The Grant consists of US$1,000 in ThriveGuide product credits, applied to access for up to four (4) families nominated by the Applicant. The Grant has no cash value, is non-transferable, and cannot be exchanged, refunded, or combined with other offers. Credits expire if unused by 3 months after award.
4. Review and discretion. Submitting an application does not guarantee an award. Awards are made at ThriveGuide's discretion. ThriveGuide may modify, suspend, or end the program at any time.
5. Applicant responsibilities. Applicants agree to nominate only eligible families, to obtain each family's agreement before sharing their contact details with ThriveGuide, and not to charge families for, or resell, Grant access. Applicants agree to notify ThriveGuide of new parent awards.
6. Family participation. Nominated families create their own accounts and use ThriveGuide under our Terms & Conditions and Privacy Policy. Participation is voluntary.
7. Not clinical or medical advice. ThriveGuide does not replace in-person therapy or professional care. It is an adjunct parent training resource and does not provide medical advice, diagnosis, or treatment. See Section 14 of our Terms & Conditions.
8. Information you submit. The application asks for organizational information only. Please do not include health or personal information about any child or family in your application. Information submitted is used to verify eligibility and administer the Grant, as described in our Privacy Policy.
9. Use of name and marks. Neither party may use the other's name, logo, or the existence of the Grant in public communications without prior written consent.
10. Liability and governing law. Sections 16 through 18 and Section 21 of our Terms & Conditions apply to the Grant, including the governing law of the Province of Ontario.
11. Contact. Questions: support@ThriveGuide.co.
